Health and Wellness Screening

Welcome to Campus!


The health and safety of our SJU Community is a top priority. To help reduce the spread of COVID-19 and maintain a safe working environment, we are asking that ALL Faculty, Administrators, and Staff complete a DAILY COVID-19 Health and Wellness screening.  Please present the completed pass to the Department of Public Safety Officer at the gate (on your mobile phone or printed in color). 
**Note that this pass is valid only for the day on which the screening was completed. Only those who receive a “Green” day pass will be granted access to campus.  


**If you inadvertently answered a question incorrectly and subsequently received a RED pass, employees MUST contact HR at 718-990-2363 in order to clear your status. Students MUST contact the Student Health Services at  718-990-2700


  • Gate 6 will continue to be open 24/7, including the walk-in gate.
  • Gates 1, 2, and 4 will be open to vehicular and pedestrian traffic from 7 a.m. to 7 p.m., Monday through Friday.

You may only create a pass on your account for YOURSELF. You are NOT permitted to submit a screening on your account on behalf of someone else. They must log in for themselves.  

** All non-employees and non-students (i.e. Vendor, Parent, etc) must also complete the screening.

There are 2 methods to submit the Health and Wellness Screening:

  1. Sign in to the mobile SJU Connect app on your phone.

    Once logged in, click on Campus Pass at the top. The screen will return to the Home page when you submit the screening. Click on the Campus Pass icon again.  A Green pass or a Red pass will appear.  A pass must be generated on the same day that you intend to visit the campus (not the night before).  A PDF attachment will also be emailed to you.  


 

2.  Login at http://connect.stjohns.edu, if you do not have access to a smart device. 

     (Instructions for logging in are found at the top of this page.)


Under Campus, click on Resources.
 

 Click on Self Screening.



The survey will appear. Answer each question, and click submit at the bottom.

 

 Once completed, an email with a PDF attachment will be sent to you.  Only those who receive a “Green” day pass will be granted access to campus. Please present this pass to the Department of Public Safety Officer at the gate (on your mobile phone or printed in color). Note that this pass is valid only for the day in which the screening was completed. 


If you received a Red pass, you will receive an email with instructions on the next steps to follow.
